Your personal contact and health information will be collected when booking an appointment. All information provided will be kept confidential unless required by law. Medical information may be shared by the various care providers involved in your care and treatment, with your express consent before anything is shared. As a solo practitioner clinic, this sharing of medical information would require a medical release form from another practitioner or a legal release of your records for legal reasons, signed by yourself before any information is shared.
